Responsibilities

• Conduct thorough patient assessments to evaluate patients’ health status, needs, and care requirements.
• Develop and implement individualized care plans in collaboration with physicians and healthcare teams.
• Perform skilled nursing procedures such as wound care, medication management, and patient education.
• Provide patient education on self-care techniques, medication management, and lifestyle changes.
• Coordinate care and maintain communication with patients, family members, and healthcare providers to ensure continuity of care.
• Monitor patient progress and adjust care plans as needed to ensure optimal health outcomes.
• Administer medications and treatments as prescribed, including IV therapy and injections.
• Document all patient interactions, assessments, and treatments accurately in compliance with home health regulations and organizational standards.
• Ensure infection control measures are in place and maintain a safe and sterile environment for both patients and staff.
• Lift and assist patients as necessary, ensuring their safety and comfort.

Skills

• At least 1 year of experience in Home Health nursing is required.
• Proficient in conducting SOC assessments and developing home health care plans.
• Skilled in wound care, IV therapy, medication management, and other home health nursing procedures.
• Strong case management abilities with excellent coordination and communication skills.
• Familiarity with home health documentation systems and legal requirements.
• Ability to educate and empower patients and their families on managing health conditions and following care plans.
• A valid California RN license and any necessary certifications are required.

SOC: $150
ROC: $120
RECERTIFICATION: $120
DC: $100
FOLLOW UP VISIT: $50
SUPERVISORY VISIT: $50
