LVN Case Manager + On Call

  • Order medical equipment and supplies as requested
  • Obtaining insurance authorizations
  • Respond promptly to urgent patient and family needs during on-call hours
  • Perform evening/weekend hospice admissions and new referral evaluations
  • Triage calls to assess patient needs and determine necessary actions
  • Provide pain and symptom management through skilled nursing interventions
  • Perform death visits and offer compassionate post-mortem care and family support
  • Document all clinical activities promptly and accurately in EMR (HospiceMD)
  • Educate families/caregivers on disease progression, medications, and care techniques
  • Communicate with physicians, RN case managers, and interdisciplinary team as needed
  • Update the coordination group immediately after each visit or critical event – this is mandatory
  • Ensure care is delivered in compliance with hospice philosophy and Medicare regulations

Qualifications:

-1-2 years of hospice and/or home health experience.

-LVN (Required)

-BLS Certification (Preferred)

-LPN (Preferred)

-Excellent written and verbal communication skills.

-Understands HIPAA

-Good problem-solving skills, able to multi-task and prioritize work quickly.

-Work requires continual attention to detail, establishing priorities and meeting deadlines.

Pay: From $2,500.00 per month
